NHS Health Checks Service Hull
Hull City Council is responsible for commissioning the NHS Health Check Programme from 1st October 2016. You will have received a contract extension notification from Hull City Council offering you a 6 month extension to the current service to 30 September 2018. The current contract is set to expire on 31st March 2018 .
All pharmacies wishing to accept the contract extension must reply to Sally Barlow via the extension letter below, to confirm continuation for a further six months under the current contract terms:

- Contractors will receive £20 per NHS health check
- A £10 premium (additional payment) is available to the Pharmacy when a Health Check is delivered to a patient identified within certain target groups (please see contract)
Pharmacies wishing to sign up to deliver this service will need to have adequate mechanisms and facilities, including premises and equipment, in place as are necessary to enable the proper provision of the programme in accordance with the requirements of the service specification.
Following a discussion with the council regarding equipment requirements, the council have confirmed they are happy to explore redistribution of Point of Care Testing machines to ensure new Pharmacies can participate in the NHS Health Checks programme (without incurring upfront equipment costs).
